Fenugreek for Breastfeeding: What It Does

Fenugreek is the most studied herbal galactagogue, and the studies are small. What the evidence shows, who should avoid it, and where it fits.

Everyone recommends it. Your aunt, the internet, the woman at the pharmacy.

Fenugreek is the herb most often suggested for low supply, and the one with the most research behind it. Both halves of that sentence need unpacking, because the second half is doing less work than it sounds like.

What it is

Trigonella foenum-graecum, a seed used across South Asian and Middle Eastern cooking, and in traditional medicine for a very long time.

It contains compounds with weak oestrogen-like activity, which is the usual explanation offered for what it does to milk production. The mechanism is not established.

What the evidence actually shows

Fenugreek is the most studied herbal galactagogue. That is not the same as being well studied.

Several small trials have found more expressed milk among Mamas taking it than among those on placebo. One found Mamas drinking fenugreek tea three times a day produced more than those who did not.

The Academy of Breastfeeding Medicine's protocol on galactagogues is measured about all of them, noting a limited evidence base and small studies. NCCIH says much the same.

Fair summary: there is some evidence, it is not strong, and it works for some Mamas and not others.

Where it fits

Before any supplement: your supply is decided by how often and how completely the breast is emptied. Milk out is the signal to make more.

A galactagogue works, when it works, on top of that. It does not replace it, and it does not fix a shallow latch, an infrequent feeding pattern, or a medical cause.

Which is why a lactation consultant is the better first step, and fenugreek is a reasonable second one.

Be careful if

You have a nut or legume allergy. Fenugreek is a legume, related to peanuts and chickpeas.

You have diabetes. It lowers blood sugar. If you take medication for it, this needs a conversation with your doctor rather than being added quietly.

You are on anticoagulants, given a theoretical effect on clotting.

You have a thyroid condition, where the advice is mixed and worth checking.

The side effects nobody warns you about

Your sweat and urine will smell of maple syrup. Harmless, well documented, and genuinely alarming the first time if nobody told you.

Digestive upset in you, and occasionally more wind in your baby.

Dose matters. What you use in cooking is unlikely to do much in either direction. What is in a supplement is considerably more.

Ways to take it

In food, as a spice. The traditional route and the lowest risk.

As tea, steeped from the seeds.

In a supplement, where the dose is higher and more consistent, and where it usually sits alongside other things.

How to tell whether it is doing anything

Give it several days to a week. Nothing herbal is immediate.

Change one thing at a time, or you learn nothing about which thing helped.

Measure the right thing. Wet nappies, weight along her growth curve, and whether she seems satisfied afterwards. Not what comes out into a pump bottle, which is the least reliable indicator available to you.

When low supply needs looking at properly

  • She is not gaining weight along her curve
  • Fewer than six wet nappies a day after the first week
  • Feeding hurts
  • Supply dropped suddenly and sharply

Worth ruling out: a shallow latch, tongue tie, retained placenta, thyroid problems, anaemia, and some medications including the combined pill.

All of those are treatable. None of them is treated by fenugreek.
